Here is a quick list of important differentials of vision loss tested on the boards:

| 3. Herpes zoster ophthalmicus | 3. Central retinal artery occlusion |
| 4. Pseudotumor cerebri | 4. Central retinal venous occlusion |
| 5. Cavernous sinus thrombosis |
Important “buzzwords” to look out for..
PAINFUL!!!!!!!!!!!!
- Acute angle closure glaucoma:
- Mid-dilated
- Non-reactive
- Increased intraocular pressure = Papilloedema
- Hard pupil
- Herpes simplex keratitis (dendritic ulcer):
- Photophobia
- Decreased vision
- Herpes zoster ophthalmicus:
- Burning around eye
- Pseudotumor cerebri
- Enlarged blind spot
- Bilateral vision loss
- Increased intraocular pressure = Papilloedema
- Retro-orbital pain
- Pain with eye movement
NOT PAINFUL…
- Open angle glaucoma:
- Gradual PERIPHERAL loss
- Tunnel vision
- Cupping of optic nerve
- Macular degeneration:
- Progressive bilateral CENTERAL loss
- Central Retinal Artery occlusion (look at image below)
- Pale optic disc
- Cherry red fovea
- Central Retinal Venous occlusion (look at image below)
- Optic disc swelling
- Tortuosity
- Venous swelling
- Cavernous sinus thrombosis
- Early vision loss
