Vision loss

Here is a quick list of important differentials of vision loss tested on the boards:

 

PAINFUL PAINLESS
1. Acute angle closure glaucoma 1. Open angle glaucoma
2. Herpes simplex keratitis 2. Macular degeneration
3. Herpes zoster ophthalmicus 3. Central retinal artery occlusion
4. Pseudotumor cerebri 4. Central retinal venous occlusion
  5. Cavernous sinus thrombosis

 

Important “buzzwords” to look out for..

PAINFUL!!!!!!!!!!!!

  1. Acute angle closure glaucoma:
    • Mid-dilated
    • Non-reactive
    • Increased intraocular pressure = Papilloedema
    • Hard pupil
  2. Herpes simplex keratitis (dendritic ulcer):
    • Photophobia
    • Decreased vision
  3. Herpes zoster ophthalmicus:
    • Burning around eye
  4. Pseudotumor cerebri
    • Enlarged blind spot
    • Bilateral vision loss
    • Increased intraocular pressure = Papilloedema
    • Retro-orbital pain
    • Pain with eye movement

 

NOT PAINFUL…

  1. Open angle glaucoma:
    • Gradual PERIPHERAL loss
    • Tunnel vision
    • Cupping of optic nerve
  2. Macular degeneration:
    • Progressive bilateral CENTERAL loss
  3. Central Retinal Artery occlusion (look at image below)
    • Pale optic disc
    • Cherry red fovea
  4. Central Retinal Venous occlusion (look at image below)
    • Optic disc swelling
    • Tortuosity
    • Venous swelling
  5. Cavernous sinus thrombosis
    • Early vision loss
